Do I have to put my hands inside when using it?

No you don’t but the Idea is you do so you can keep the massager in place on your neck and shoulders as the rotating balls are doing their work. It will move about and slip off the shoulders if you don’t hold it in place. It’s comfy to wear. Alternatively it will work if you place the massager over your legs or back when laid flat. You wouldn’t need to hold onto it then.
