Is this cushion only for indoors or will it also work in a car?

The one i got came with a wall outlet plug and a cigarette stlye plug ,so yes it could be used in a car or an office chair

How long is the cord?

Eyeballing it, maybe 3 feet (to plug into the wall). It is much shorter, though, if you use cord that plugs into a car lighter port, though.

In a car can both seat heaters operate at the same time

I would not recommend this. The seat heat gets quite warm, we had a blanket underneath the seat and it got a burn mark on it. With two going it could cause either a burn mark or perhaps some melting of material.

Does it work while plugged in or does it have to be charged?

I keep mine plugged into my car @ all times. It doesn't work unless plugged in. It's great - I've had mine for over a year and love it!
