What are the clamps for?

They are used to clamp your sheet, blanket, or whatever bed linen you plan to drape over the lifter to minimize the possibility that it/they will slip off wheb you move.

will this work with a hospital bed, and with the knee lift raised?

It will fit between the mattress and the foundation. If the knee is raised, it may not lift as high, but it is adjustable. My bed lifts the foundation, but only the foot and head moves so the blanket lifter moves with the foot. You must simply look and see how your bed moves. I would assume it works the same way since a mattress cannot "move" by itself. My bed lift has a small retainer at the foot to hold the mattress in place. I had to move the blanket lifter to the side slightly for it to fit. Ask more detailed questions if you want more info.

How tall can it get in the horizontal position?

You can't use it in the horizontal position. It will go up to 36 inches vertically. How high off the bed depends on how thick your mattress is.
