How does it stay in place is there a way to secure it other than putting it under the bottom sheet ?

I don’t think there’s a way to secure it without putting it under the fitted sheet. Maybe you could put it over the sheet using strong double sided tape or something that would secure it, but it works well going under the sheet. It seems to stay in place.

Yes, it is flexible and bends with the bed.

Would this work if placed on top of the sheet?

Yes it should, how well I never tried. I can say I was skeptical of the idea but my wife who uses it under the sheet can't be happier. I was also concerned that putting it under the sheet was not going to look or feel right...turns out it easily goes under the sheet taking up a minimum amount of space with almost no interference when sleeping. This product works VERY well and I woul recommend to anyone with a problem of rolling out of bed. Been using for about 2 mo with zero incidences or complaints. *****
