Q: Does it come assembled?

No, but it was not that hard to assemble. My husband did it by himself.

Q: How large of a cup will the cup holders hold?

Did not attach the cup holder. The chairs are outstanding

Q: I had knee replacement so I need a higher chair that is easy to get out of . Is this chair easy to get out of or is it pretty low

The chair itself is built well and is good and sturdy. It is NOT a high sitting chair, it is lower sitting than many rocking chairs. It just all depends on how much pressure you would have to put on the knee in getting up and down. I would say that as you knee gets stronger that will not be a problem getting in and out.

Q: Has anyone found seat cushions that fit this rocker well?

I found perfect cushions at my Goodwill store.
