Pilot Extra Fine IS really fine. This is the Acroball ink for the Center of Gravity pens, a cushy big bodied low center of gravity, slightly weighty pen, that should allow for long writing tasks with the least fatigue problem. The only drawback is having to order the extra fine refill from desertcart, but fortunately it is available. It's the one you want. But now it's available in the 0.7mm FINE too ( BRFN-10F-B_ for black), my favorite. The CG pens are spoiling writers, ruining our love for all other pens. Needed a refill
It fits and it works.

Not an exact replacement
I’ve been struggling to find the replacement ink cartridges for my favorite Dr Grip breast cancer pen (white with pink). While these fit just fine, it is a bit finer in size. My original didn’t make as fine of a line and felt like it rolled a little more smoothly. But overall these will work and be good replacement.
