Do you need a box spring? Or is the mattress fine to sit on the floor

Floor is the way to go, a box spring would probably be too tall.

What country was this bed made in and does it contain any lead at all?

The box said it was manufactured in Vietnam, unsure of lead,the hardware was unlabeled.

Anyone have a problem with screws not being long enough?

Yes that and one section is flawed where the piece goes to keep the half rail together in front. Screw is too long and poses as a scratch risk.

Are the safety guards removable?

The way the bed is assembled, I’d say every piece is essential for structural integrity. Nothing is removable.
