Q: How many Mounjaro pens will this hold?

It will hold 4 in the straps, and then you have room where your cool packs go. It also expands to give you more room. Your needles, your accu check strips, your insulin needles, and your meter will fit with no problem. I love mine and would recommend it for anyone.

Q: will this fit Repatha pens and how many ?

I'm not sure on that particular pen but it does fit a novalog pen and a lantus pen with room for two more pens. Sorry I couldn't give a actual answer.

Q: Can I purchase additional ice packs?

I just saw some diffrent ones you can get called youshare ice pack. Dont know if they will work as I did not read the details but check them out if you are still looking.
