It's easy to understand and fill out without "lawyerspeak." Two members of our family were in an accident recently and were in a coma. Neither of them had a Prime Directive, both died of their injuries, so it was left up to the family to make all the decisions. It's been a heartbreaking experience. The blow would have been a little softer if their wishes were known to us.

What I needed

Recently ordered a living will kit from Permacharts through Amazon. For unknown reasons 1st shipment never arrived. Permacharts kindly re-shipped item and it too went missing. Seller sent online copy for me and credited my Amazon account. The re-shipped item showed up in the mail today, one month from mailing date. I'd be happy to repay $10.95 to Permacharts as it was obviously USPS that lost envelope.

I think the forms were perfect for the most part some of the lines could be longer here or shorter a bit there however they are easy to understand filling them out and covers all the basics for a Living Will with extras such as DNR instructions as standard and if you don't want antibiotics and room for added instructions enough for notes on other aspects of care or no care. And its overall compared to similar more expensive items a fine product. And you get two forms to fill out in case you have a loved one who needs one or make mistakes on the first form.
