Does this work for Cockroaches ?

I really don't know! I only had ants & it worked great for them!

What is the black square that comes with this? There are the 4 traps as stated and then another black piece.

I got two of those black pieces. I never used either. I think they're stick up bait traps. I couldn't find anything in the post explaining them. fyi - These didn't kill my ants.

How long are these good for once left out?

I'm not really sure. All the ants were killed when I first put the traps out & there hasn't been a sign of ants since. I have a friend who uses them also & she said she changes hers once a month.
