Q: How to decide on the height of the wedge? 8 0r 12 inches

I think it might depend on your sleep position - I am a side sleeper and would have difficulty accommodating more than an 8" incline (and sometimes wish it was a 6" incline). Either one should work if you're a back sleeper, particularly if you're someone who can fall asleep in a chair watching TV!!!

Q: Can this be used under the mattress?

No, mattress would be too heavy. Surface of the bed only.

Q: I'd like to buy some additional covers, maybe in colors, all cotton. Suggestions on where I might get some/what to look for? thanks

I had planned to make additional covers, but then only ended up having to use it a few nights. I think I would have just wrapped some cotton fabric around it and secured it with safety pins on the bottom side. It stays in place really well, so I don't think it would shift around with the fabric pinned on.

Q: Did anyone buy a cover for this that fits?
