Does this costume include boots?

Not actual boots. Only "boot type" covers. You will still need to wear your own shoes.

Does this costume run small? Would a medium fit a 30 inch waste

The waist on the Medium size costume fit me perfectly and I wear a size 32 waist. There is plenty of stretch. If you are a 30 inch waist you should be fine with a 'M'.
