Would a monitor dvi cable fit through the opening? does the top cap come off?

Yes it will fit and yes the top seperates.

How many cords will fit through this grommet?

The top of this grommet is a cap, it comes off; run your cables through the larger hole, then replace the cap. I have one thick power cable and 3 thinner comm cables through one of these, and they fit fine. If you leave the cap off, you can put many cables through this.

does it come with 2 or just one

There is a single 2 and 1/2" grommet. The top portion will come off so you can mount the grommet and to make it easier to thread large plugs through. But there is only a single grommet in the package.
