I would say maybe a 5XL. I don't wear a 38DD but I'm 5'3" and I also wear a size 14 in pants/shorts. It fit me a little big around my boobs but it should fit you perfectly :)

I would say a medium my daughter has a 20 inch waist and she got one small and one medium but both fit her, she just likes the medium better.
