Q: what watt bulb should I use? The one provided is too hot and the wax is breaking into small clumps/balls

This lamp uses R39 E17 25W bulb. 25W is already the smallest watt bulb for this lamp. Usually there are two reasons for small wax clumps: (1) the environment temperature is too high. The best environment temperature for this lamp is at 65° to 70° F. If the temperature is below 65° F, the wax drops may not be able to form or take longer time. If the temperature is much higher than 70° F, small wax drops may form. (2), the lamp is running more than 10 hours. Use the lamp for less than 10 hours at a time for best results. Allow it to cool to room temperature before using it again.

Q: Why does the liquid look black in the picture?

The liquid is clear water. The picture shows the lava lamp in the dark environment. When in bright environment, the liquid will be transparent /clear water.

Q: How can this have 4 stars when the bulbs come used & the thread is bent in the device making it unable to use after first Bulb dies?

You received a defective or returned lamp. I ordered the same one and it was new and has worked just fine.
