Brilliant book. Has a tick list and even shows you how to fill in. I bought this as I'm a childminder and this book allows me to give a copy to parents too.

Used this as a sporting accident book for children Clear and easy to use . Tear off copy to give to parents / guardians and one copy to keep on file . Just what we needed at a reasonable price too. Only one downside is the size A4 it a little awkward to keep in my sports bag however it is a perfect size to write on .

I like this because it has a picture of the body front and back that you can circle the area injured. This makes it clear rather than describing it. Arrived within 24hrs.

We needed a new workplace accident book, this one appealed as it has an integrated body chart to mark the location of an injury rather than having to write copious amounts of notes! Ticks all the boxes for us and handily has a duplicate function so we can forward the hard copies of the accident reports to the relevant department and maintain a copy for our own records.
