Q: Does this come in a double outlet option?

It comes with one duplex plate (screw in middle) and one decora plate (rectangle, one screw top and one screw bottom).

Q: the outlet place appears to be slightly longer than the opening on the shelf. Is it supposed to be this way?

Yes. There is a large enough hole through the assembly to feed the cord through and coil it around to hold it in place.
