Does this allow me to measure the light on a far away object using my L478?

yes. the "spot sensor" is small. You can meter on the tiniest detail. Used it for Large Format shooting 4x5 film in B&W mostly at f~64 and frequently metering very deep. Worked perfectly.

So is this the adapter to make it a spot/reflective light meter for it to read Middle Gray?

This viewfinder will enable spot metering(reflected light measurement).

You get what you pay for. If you want cheap you'll get cheap. This is part of a quality tool. perhaps if you look long enough you can find it used on Amazon or E bay.
