How would one decide what length to purchase?

I would suggest a sewing tape measure. You'd want them to wrap around your wrists at least twice when stretched. At least, that's the way I find them most supportive and comfortable.

Can you wear training gloves with this?

If you have the 18" or 24", they'll wrap around your wrist twice and be pretty thick. You'll probably have a hard time getting the bottom of the gloves to fit over the wraps.

Is the elastic part that goes around the thumb tight, that can cut blood circulation?

Not at all, I have been using the straps for a couple of months, they give you great support. The elastic as new is tight enough and with time it loose a bit.
