After careful diagnoses, I determined that it was the starter, or at least the solenoid that was attached to the starter. I bought this as a cheap install, and the intermittent issue disappeared. There is however a whine at start-up. I'm not sure if it needs shims, or if it's a manufacture defect, but the car does reliably start whenever I turn the key to the crank position.
