Can someone give me a general estimated idea of the cost to assemble this assuming a table top size of around 5'x2' or 3'?

I spent $65 on 2*4s and $80 on 2 sheets 4*8 3/4 sanded plywood. That's all I really needed but I decided to upgrade the screws, stain and poly the wood so that's up to you and the cost varies.  Bare bones bench you're looking at $150. Mine is 2x8ft and totalled just over $200

Is there anyway to attach caster wheels to make this easier to move?

YES! In fact it is quite easy to do , "Adding Castors/Wheels", to this workbench. As sturdy as this bench is once built it is easy to flip on it's side or back. And in that position simply attach your wheels. Just make sure you purchase GOOD wheels! And with a good mounting system. I at first tried to save a buck and bought some really cheap wheels from Amazon.  Thought the wheels were rated at 500Lbs. That must have been a one time use rating! I then found a deal on Amazon for "Locking, Two inch dia., Polypropylene wheels with metal frames", Rated for 1000 Lbs. and they worked great. "STAY away from any plastic wheels regardless of the ratings!" The catch is the mounting system for the wheels. Most either have four holes you can place screws into OR a single steel 'Round", post with a metal "O-ring" to lock it in place. The latter you can order a larger plate that will attach to the bottom of the bench that the post slides up into. These worked for me. The one's with the four screws? Even with longer screws than recommended, -"4 x #8, 3-1/2" inch long, metal deck screws" they failed repeatedly. The bench weighs allot by itself and the four screws literally tore though the bottom of the bench! The wheels with a 3/8" inch metal center post worked with the mounting plates I made from heavy steel angle, "L-Brackets from the hardware store", with the 3/8" inch hole that I drilled in them for the post. With four 1" screws into the bottom but also two into the side. So that they were held both vertically and horizontally. Good luck!

Question 3

Are the shelf supports included with this kit the same size as the 90124 Shelf Link units ( which accept 4- 2x4 in width) sold separately? From the pictures it looks like the ones with this bench kit are using 3 - 2x4, or one narrower.

I ordered the bench kit (90164) and the shelf link kit (90124). I did not use the shelf kit supplied with the bench kit on the bench itself. I combined them with the shelf link kit to make more shelves. They are exactly the same parts. I hope this helps.

Question 4

I see in the instructions that they state to put a piece diagonally to support the work top.  Was there a place in the leg to attach this piece?

Yes, the upper leg support accommodates the (3) pieces of wood . The left & right sides of the wood are screwed into the leg support and the diagonally placed 2 x 4 is just laid diagonally across (sits on the upper leg bracket).
