I have two children to buy for; 3 yr old head circumfrence of 18.5" & 6 year old 19" - so the small works for both?

Can't really answer your question. I have a four year old, who is big for his age and it fits him. If I remember correctly, the information about the helmet says 3 to 7 yrs. You'll have to try it for yourself.
