Thought provoking
A superb book, well written and filled with valuable lessons.As a safety professional I am routinely disappointed by the immediate leap to blaming individuals following an incident, particularly those incidents with serious consequences. Armed with the lessons from Reason, Weick and Hopkins, I have long pushed and driven organisations to take a more holistic view during the investigation process, however this book had me questioning and challenging my own methodologies and preconceptions.A must read for anyone who has the desire to truly understand accidents and the conditions that might increase their likelihood.

It is an excellent analysis of the incident
I used this book in teaching a Human Factors Engineering course. It is an excellent analysis of the incident. I find this incident and the analysis of the USS Vincennes of an Iranian airliner some of the absolute best cases for teaching how explicable things happen and why. The only disagreement is the author somewhat downplays the desire of the pilots to get a kill. Although talked about I think it may have been more of a factor than it is given credit for.

Powerfully relevant for complex organizations
Many useful insights into the dynamic processes that will inevitably lead to catastrophic failure. Scholarly, well researched and well presented. At times repetitive but the "ah hah" moments are worth the effort.

Came in near perfect condition!
I was pleasantly surprised when I received this book a month ago. I expected it to be a little battered since I wasn't ordering it in "new" condition (which I was okay with) but when it came in the mail it looked brand new. No markings on the pages or anything! Great buy and also very insightful book.

Outstanding detailed review of the investigation, and new interpretation of the findings.
Scott Snook provides incident investigators with invaluable informatin on 'normal accidents' in highly reliable organizatins that goes beyond the typical 'operator error' or equipment failiure conclusion to real system issues. A must read for those involved in root cause analysis.

Practical drift/sailing by example
This book was referred to me by a lecturer when I was struggling with making a rule based workplace. It took me over 4 years to read the whole book as I initially only read parts relevant to what I was doing at the time but reading the book has given me a lot of insight into managing safety inn a highly restrictive environment. A great read for all the tit-bits of info

A comprehensive analysis
This book takes a single fratricide incident and analyzes the contrubutory factors. It is convincing, well written, and beautifully crafted.
