Can this be used with the iPhone XR?

yes I have the XR and works perfectly. Keep the base station as near as you can to the child with the temperature reader on and pair it with your phone. You must keep the app open on the background or it will stop your readings ☺️

Could an adult use these without patches for a quick reading?

Officially: no. Unofficially: It takes 8 min to warm up, while it is stuck to your body, so I wouldn't call it quick, but I don't see any reason why you wouldn't be able to use it without the patches (you may have to cover up the other electrodes for charging and i would stick it on my skin with some tape, but it could probably go in your bra too.

Can the child wear this when awake?

Yes - it sits under their arm near the armpit very sticky
