Q: Does it need the car aerial to work?

Absolutely not. Tune the car radio to the exact frequency shown on the digital display and the device directly transmits to the radio on that "station" I personally found that the preset frequency on the device had too much interference, so I changed it (full instructions on how to are included) It's a very good clear signal on 87.5 No aerial needed at all

Q: My phone connects to unit, but I cant hear people on the other end?

Make sure you are connected to the correct FM radio and that you have calls and audio enabled for the device in the Bluetooth settings. Try turn the volume up on your phone/device/radio
