It's about 5 inches and it's very effective.

Does the desk need drilling for the fastening screw?

No. No drilling needed. The screw has a flange base that tightens flush against the table. It's a strong enough join to hold a full large cup of coffee and other weight.

what is the distance (length) inside the table and the length outside the table when fitted?

It sits quite close to the edge so, id say 2” on the desk and the remainder of the support overhangs. If you have it in line with your forearm, its apmost to your elbow
