Are all the fixing screws included?

As I recall, yes screws are included. But as always, they may not suit your particular application.

Can these be used as inset hinges? Or overlay only?

Overlay Only. You need a different type for inset.

Are the mounting plates included?

Everything you need is supplied: you just need to screw the stuff to the frame and door. If you are replacing existing hinges that are broken, then pre-existing holes may not be in the right place. Otherwise, fairly straightforward.
