Is the plastic lid BPA, phthalates and lead free?

Thermos never use BPA or lead products and let me say this flask is good keeps hot for hours.

Will this fir in van cup holder

It probably depends on the van. Mine will not fit in the dashboard holders on a vivaro/traffic but it does fit on the middle lower and inside the pull out pocket to the right of the steering wheel.

I have just received the item, and it's saying 9 hours Hot and 12 hours cold. This is different from the advert, which is correct?

The amount of time is strongly related to the temperature difference. If the drink is at 80 Celsius and ambient is 20 Celsius then there is a 60 degree difference. If the drink cools by 30 degrees to 50 Celsius then you might say the drink is no longer 'hot'. If the drink is at 1 Celsius and ambient is 20 then there is a 19 degree difference. If it warms to 15 Celsius then you might say it is no longer cold. The latter temperature change will happen slower because although the amount of change is smaller, the starting temperature difference is greater.

Does need a decent grip to open.
