Yes you do, unless you are using it in the car then you use the connection provided. This has been a really good purchase

Has this massager got a guarantee as the last one i had packed up after 2 months

I don't know about any warranty or guarantees as the one I've got works well still. Maybe Amazon will help. Or message the seller. Other than that, I don't know, sorry !

Will this massager work in a car?

If you have a small inverter it should...but it's quite bulky so if you have no space to move seats backwards then you may struggle. I would give it at least 4 inches for seat to slide back. Also it could interfere with driving if you are not used to it because the massage is quite brutal.
