Yes, it works well for G903, 2 pcs in the package. Not 4.

Is this a pack of 2? Picture says 4

When I ordered this it was 2. However I see that picture and description says 4. I would contact Amazon customer support to verify your concerns. PS: cheaper ones are suggested to me get Japanese version of these they are of higher quality.

What is the lifespan of the d2f-01f-t? i.e. what is the number of actuations it is rated for?

thought I read it was some 5,000,000 actuations. The mouse I used them in is over 15 years old. The originals lasted a long time.

Will this work with a MX518 gaming mouse, and are these still made in Japan?

The ones I received said they were made in Japan. I believe the mx518 is the same size as my Logitech mouse. You shouldn't have a problem installing these into it.
