Looking for a cover to protect my sons mattress from bed wetting. Will the urine leak through to the mattress?

I want to say it won't leak. I have the mattress protector on the trundle part of a day bed and the day bed. My dogs sleep on the trundle pull out, when the day bed is in use. One of my dogs threw up on the trundle and the mattress was fine.

is it noisy?

No, it's not noisy at all. I have both the box spring and the mattress for my queen sized bed covered, and there's absolutely no noise. I don't even know its there until I change the sheets. I should also add that it might seem to be superfluous if you have no reason to think you'd have a bed bug problem. I would not have even thought of it until I read the terms of my lease. It expressly stated that the property owner would have no liability should there be any issue with bed bugs.  I have never experienced bed bugs, but when I read that disclaimer, it occurred to me that I was moving into an apartment that the previous tenants had lived in. Suppose the prior tenants introduced bed bugs via their mattresses? That would be my problem, not the property owners, under the terms of my lease. Although the magnitude of the risk might be low, the consequences would be terrible, and it would be entirely my problem to solve, under the terms of my lease. So if it turned out there were bud bugs left from a prior tenant, these products would at least ensure that the bugs would not be able to take up residence in my high quality mattress and box springs. Fortunately, there has been no evidence of bedbugs, but I still keep the covers on. They also keep dust from soiling the box spring and mattress over time.

Question 3

Is this vinyl because it's for a bed for a patient in a Nursing Home

There is a vinyl version that also sells for the same price, but this is the 'polyester fabric' version , which is basically microfiber with a thin soft rubbery sealant on the back of the fabric on top. This does repel dampness and sweat reasonably well, but I haven't tested it against large spills or bodily fluids, yet. I do have this on my own bed, and love the feel of it. It's a soft fabric that doesn't crinkle like the vinyl does.

Question 4

I have a king bed with a split box spring.  Should I get the king or two twins for the box springs?

You should get Twin XL, not Twin. The split box springs on a King bed are Twin XL. Hope this helps!
