Works for me - but might not work for everyone
One of my knees is very sensitive to foot angle after having recovered from a fracture some some 15 years ago. I found a pair of bike shoes that worked well for me at that time, and bought 5 pairs. I just used up my last pair, and couldn't find a bike shoe with the perfect footbed angle for my knee - I do imaging it is out there but it's hard to tell without actually attaching a cleat and riding a few miles. To cut to the chase, using this wedge allows me to make the foot angle almost perfect, or maybe as good as it can be. It worked for me, but I wasn't sure whether it would work, and was hesitant to shell out for this rather high-priced item without prior assurance. Maybe it will work for you and you won't regret the price, or maybe it won't work for you and you won't get your money back. You have to pay to find out, without prior assurance, but if your problem is serious enough it's worth the gamble.

Great if you need to adjust your foot angle and have a bit of extra room in your shoes
If you need to angle your foot and you have a little excess room in the front of your cycling shoe, these are a great solution. My shoes were a little loose, so I could get two stacked up under my insole. They fit perfectly in my size 45 Shimano shoes, without having to trim them. I need about 6 degrees of correction, so I also had to use three cleat wedges. Overall, I like the shoe inserts better because the cleat wedges are more laborious to install, and not very durable (they get deformed when you tighten the cleat screws.) They seem overpriced, but I suppose they have to have the wedge-shaped plastic sheet stock custom made...and since it completely got rid of my knee pain, I'm not complaining.

Working with a professional is always better, but if you understand pronation and the importance ...
After tons of experimenting with Bikefit cleat wedges, I ended up using three cleat wedges in one shoe and 2 in the other. But the 3-wedge shoe just wasn't right. I added an ITS wedge and perfection! Total linear mechanics while cycling know! I advise testing in front of a mirror whike on a trainer. You need to feel and see no lateral knee movement.I also use two ITS wedges in one running sneaker and one ITS wedge in the other, in addition to one ITS wedge for each HEEL. Just trim to fit the smaller heel area of the sneaker. They go under my Superfeet Green. It takes a lot of experimenting, patience and an honest observations to get the right fit. Try one at a time and take photos/videos and write down your observations.Working with a professional is always better, but if you understand pronation and the importance of linear mechanics, and you only ride/run recreationally, you can do this yourself. For anybody who rides/runs hard, you'd be crazy to risk your joints to save a few hundred dollars on a bike fit or custom orthotics.

Correct foot alignment to address knee pain
I use cleat shims on my road shoes with Speedplay pedals, but cleat shims won't work so well with the recessed SPD cleats in my mountain bike shoes with the Shimano XT Trail pedals. At first I didn't use shims on my mountain bike but I started to have some minor knee pain after long rides. I put these inside my shoes keep my feet at the proper angle and so far it has helped to eliminate that knee pain. The ones I received are green rather then the red shown, but you can't see them after they're installed anyway.

The amazing solution to my knee problems!!!!!
Someone recommended these to me because I was having so many knee issues while cycling. I spent like $200 on a bike fitting, got a new saddle, tried correcting my form, adjusting my seat height, but NOTHING worked. I was very limited by the amount of riding I could do. When I ordered these online and received them in the mail, I was pretty cynical. But after putting them in my biking cleats and going for a 40-miler, I was pain-free for the first time. I didn't want to get too excited too soon, so I just kept going on long rides to see if the pain would come back. It didn't!!!!! Thanks to this product, I will be doing my first century ride, which I NEVER would have been able to do with my knee pain. I just did a 68-miler today (with hills) and have ZERO knee pain!!! What a miracle!!!!! Whoever invented these is a GENIUS!!!!!

Okay?
They work—if you need them. I think a good pair of insoles usually does the trick, though. And, unlike these, they’re not overly rigid and won’t skip around. Meh.

Numb toes
It might be fine if you have room in your shoes. They made mine too tight and made my toes go numb.

Pricey but they do their work
I bought these wedges because I was suffering from a knee pain when I ride on bike for more then 16 miles. I have flat feet and was having foot pronation. So that was making my knees unaligned during my pedaling. I received these wedges a couple weeks ago and made a bike ride of 33 miles without receiving knee pain. Thus, so far I can say they do their job. Even though inside shoe wedges occupy space on the toe box, I preffer them over the cleat wedges because those tend to cause the cleats to slide. I find them a bit pricey but if I had to buy them again I surely would.
