What are the dimensions of the folded envelope?

Our item can make notches and corners for many sized envelopes, but the envelopes in our listing illustration are 3" x 4”, so you can use 6” x 6” card-stock to make these kinds of envelopes.

Does it come with instructions on how to make the envelope, like how to figure where to make the score lines?

No it doesn’t come with instructions, mine broke after the first use. I would suggest getting the Envelope Punch Board 6.75"X10.5" by We R Memory Keepers has the same punch as this one your looking at but also has a place to score your paper and it comes with instructions. Hope this is helpful
