It is static. Also, it’s not very bright at all so the only effects you can see are looking at the bulb itself. Still a very cool bulb though!

Does this bulb require a certain type of lamp or fixture? I've tried 3 different ones in the dark, and the bulb isn't working

No, they work in any standard size lamp socket. Did you possibly buy any items from the USED section? They may be damaged. Best of luck, I hope you get a new set of bulbs that work properly. These fireworks bulbs are a lot of fun!!

Can this be used in a ceiling fan light?

Yes, item# ST19/PRISM/LED can be used in a ceiling fan, as long as the base matches the ceiling fan's socket and the wattage the bulb uses does not exceed the ceiling fan's maximum wattage allowance. Of course, please keep in mind the size of the bulb as well, if there are any physical restrictions preventing the bulb from being screwed into the socket properly it should not be used.
