You're correct. The Medium size seems to have been discontinued or at least suffered a many years long shortage. They are still available but at an extraordinary mark-up, which is baffling. They used to sell them at my local stores but now only carry the Women's, and while you can get the larger size Men's you are still trimming a half-size larger and the arch and shock padding are slightly different locations. It's pretty unnerving as a mailman who walks 14 miles a day to have finally found the perfect product only to now have to pay upwards of $40 for a pair vs $13 for the same product but slightly different sizing.

Are these inserts thinner in the front half than the sport gel inserts from Dr. scholls? I need arch support with ultra thin front/toe half.

They are a little thinner, but not ultra thin. I found a pair at Walgreens that were excactly what you may be looking for. I think they were Dr Scholls also, made specifically for high arches. Not necessarily for running, but they are very cushioning and use them for running & elliptical workouts:)
