It is made in Poland. Yes it does include glycerin. Here is the list of ingredients: Aqua (Water), Paraffinum Liquidum (Mineral Oil), Hydrogenated Polydecene, Cera Microcristallina (Microcrystalline Wax), Paraffin, Glycerin, Steareth-21, Theobroma Cacao (Cocoa) Seed Butter, Steareth-2, Cetearyl Olivate, Sorbitan Olivate, Panthenol, Olea Europaea (Olive) Fruit Oil, Tocopheryl Acetate, Lecithin, Ascorbyl Palmitate, Ascorbic Acid, Cetearyl Alcohol, Hydrogenated Olive Oil Decyl Esters, Sodium Polyacrylate, DMDM Hydantoin, Methylparaben, Ethylparaben.

How long is the shelf life of the product?

I really don't know. I had mine for at least 8 months. I used it everyday. I would imagine that the shelf life is longer than that.
