Do you need to unscrew the bottom of candle in order to change the battery?

You do not need any tools to change the battery. There’s a plastic tab you can open with your finger in the bottom. Each candle uses a single CR2032 coin cell battery.

if I use 20 of these (on tables) in a room...is the flickering going to be too much?

Well, think of 20 real votive candles flickering in the same room - if that would be too much then maybe this would be too, but I don't think so. You could also always use less candles!
